Local isolations ona system

One for the engineers, did we used to take an isolation ourselves as we are next to the bit of kit and it cannot be started from elsewhere to change say a flier on a low pressure fresh water system? Asking this question as I now work in the power generation and we are discussing the requirement to isolate and issue a permit to work on a water system, where if we got it wrong, you get wet!

Label Air lines on valves as well one supply and one Exhaust or better still use coloured tube,throught the ship. Say blue supply and red exhaust/feedback/flag indicator. Also teach personnel not to remove all bolts/ clips until you give it a Tap to break the seal, and observe that the flow reduces,B4 removing the last bolts/clips.
