Sounds awfully like you have something to hide I'm afraid.

Get yourself registered, speak to the homeless charities if you have no fixed abode and need to register with a GP.

Unless you are ten years old & were never registered, you have an NHS Number and with it, a medical record somewhere.

Nah nothing to hide at all what gives that impression?
Generally, those that use implausible excuses for not providing the necessary information, have good reason.

Clearly that doesn't apply in this particular case, so registering with a GP as a homeless person will not be an issue.
NHS said:
You should not be refused registration or appointments because you don’t have a proof of address or personal identification at hand. It is not considered a reasonable ground to refuse registration. This also applies if you are an asylum seeker, refugee, a homeless patient or an overseas visitor, whether lawfully in the UK or not. http://www.nhs.uk/NHSEngland/AboutNHSservices/doctors/Pages/NHSGPs.aspx

Go back to the charity and explain your circumstances, you'll find you are not the first to encounter this problem & there's a mechanism in place. As a minor, you are considered vulnerable and will undoubtedly be given specific guidance. If not. pm your contact details & I'll ensure it .

Not at all.

Procedure to change what specifically? Address? Just update your AFCO.

In my experience the most difficult issues when processing homeless applicants is obtaining verifiable identity, maintaining contact and gaining security clearance. Medicals tend to be relatively easy.
