How do you properly address an officer?

When addressing an officer do you address them by rank or would you say “sir” or “ma’am”?

For example would a commander bollock a midshipman for being called sir?

A Raleigh GI was overheard addressing one his trainee Middies by prodding him in the chest with his pacing stick; 'Sir, there is cnut at the end off this stick. He is not at my end of this stick.'

Not only so old that it was found carved into the Victory, incorrect as middies don't drill at Raleigh.
