The Navy has launched two formal investigations to determine why the vessels collided

I can help them out there....

In my humble opinion, I think that the two vessels collided because they were too close to each other !

That should save them some time and money on formal investigations

So far so good Scrumpy but you have overlooked the follow up question.
What were the circumstances which brought them so close together they collided. :?:

In the old days there used to be a comic circulated annually to Officers called 'Collisions and Groundings' which chronicled the more juicy of the S232s received by their Lordships during the year. The Persian Gulf I seem to remember was a fertile feed for this, due partly to the sandy bottom not always staying where Droggy had last found it.
